Providing data is a condition for receiving instructions on how to use the medical device.
The data will be processed for the purpose of receiving instructions on how to use the medical device, which contains information necessary to identify the product and its manufacturer, as well as any safety and performance information relevant to the user or another person, as the case may be, in accordance with the Regulation of the European Parliament and of the Council (EU) 2017/745 of 5 April 2017. on medical devices, amending Directive 2001/83/EC, Regulation (EC) No 178/2002 and Regulation (EC) No 1223/2009 and repealing Council Directives 90/385/EEC and 93/42/EEC (OJ EU L. of 2017, No. 117, p. 1 and subsequent amendments).
The legal basis for processing your personal data is Art. Art. 6 (1) (c) of the GDPR in connection with. Regulation of the European Parliament and of the Council (EU) 2017/745 of April 5, 2017. on medical devices, amending Directive 2001/83/EC, Regulation (EC) No 178/2002 and Regulation (EC) No 1223/2009 and repealing Council Directives 90/385/EEC and 93/42/EEC (OJ EU L. of 2017, No. 117, p. 1 and subsequent amendments).
The data will be stored for no longer than 30 days from the date of their submission.
In relevant cases, recipients of the data may be entities authorized to do so under the provisions of the law.
Everyone has the right to request access to their personal data, their correction, deletion, processing restriction, as well as the right to lodge a complaint with the supervisory authority (President of the Personal Data Protection Office) if they believe that the processing of their personal data violates the provisions of the GDPR.
The data will not be transferred to a third country or international organization.
The provided personal data will not be subject to automated decision-making, including profiling.
